If you complete the activity and the course evaluation without exploring the additional computer links, you will earn 1 credit. If you follow the links to your sub-specialty to see what is required to fulfill the recertification and to get reimbursed through Medicare, you will earn 2 credits. If you then complete the Performance Improvement Project, you will earn an additional 5 credits for each Stage and a bonus of five for completing the entire project and the evaluation for a total of 20 additional credits.

Objectives:
Upon completion of this web-based course, the participant will be able to:

1. Create an office team to improve clinic patients’ health.
2. Develop an office system implemented by the team to improve patients’ adherence to nationally developed guidelines for prevention or treatment of their diseases.
3. Identify office performance gaps for designated conditions.
4. Create a system to follow patients with that condition to measure improvement in achieving recommended goals.
5. Develop a team approach to improving the performance gap.
6. Record systematically patients with the designated condition to identify improvements and barriers to patients’ success at least twice over a specified interval.
7. Evaluate the system with the team to document successes in and barriers to narrowing the performance gap at the end of the project.
8. Apply lessons learned to improving another performance gap.

Biosketch : Dr. Kathleen Davis is a Board Certified internal medicine physician with over 20 years experience in clinical medicine, health education, and medical practice management. She has worked in various health care settings in the U .S., Uganda , Guatemala, and Laos . Dr. Davis earned her Diploma in Tropical Diseases and Hygiene from the Liverpool School of Tropical Medicine in 2007. From 1996-2005, she was a clinical attending and Co-director of Outpatient Graduate Medical Education for Madigan Army Medical Center .
